Most finance leaders inherit a revenue cycle rather than design one, and the numbers that matter get buried under the ones that are easy to pull. Gross charges look healthy, a clearinghouse dashboard shows claims going out, and the monthly deposit lands close enough to budget that no one asks harder questions. The gap between what was earned and what was collected rarely announces itself. It accumulates quietly, one underpaid line and one abandoned appeal at a time.

The measures worth watching are unglamorous. Net collection rate tells you what share of the money you were actually owed made it to the bank. Denial rate, broken out by payer and by code, tells you where the friction lives before it becomes a write-off. Days in accounts receivable and the shape of the aging buckets tell you whether cash is moving or quietly stalling. A partner who cannot produce these on demand, in a form you can read without a translator, is a partner asking you to trust the deposit and look away from the leak.

Outsourcing is not a decision to hand off responsibility, it is a decision about where accountability lives. The question is not whether a vendor can submit claims, almost all of them can. It is whether they will work the hard ones: the carve-out routed to the wrong entity, the authorization that lapsed mid-treatment, the denial that everyone else files under too-small-to-chase. Those are the claims where specialty revenue actually leaks, and they are the ones a volume shop is structured to ignore.

Before you sign, read the partner the way you would read a set of financials. Ask what they measure, how often they report, and what happens to a claim after the first denial. Ask them to walk one aged account from stall to recovery. The answers tell you whether you are buying a mailroom that forwards claims or a team that goes looking for the money others have already given up on.
